Attendance Policy: Please read thoroughly.(Required)

I understand each dancer is limited to three absences before January 31, and three absences before the beginning of May, and any dancer exceeding this limit will be expelled from DE. Unused absences cannot be carried over.

Performance Eligibility Policy: Please read thoroughly.(Required)

I understand my child is required to participate in the Dance Competition, Holiday Magic, CADT Annual Concert, and all dance school recitals. For my child to perform at an event he/she must be in attendance at the class/rehearsal directly preceding that performance.

Dance Etc. II/DE: Select the payment option of your choice. Payments will be drafted from the debit/credit card on file so be sure it is up to date in the Parent Portal.(Required)

One Annual Payment Agreement(Required)

Please charge the card on file. Note: If registering online, your account will be charged the smallest installment rate. The remaining balance will be charged within 48 hours of registration. By submitting this form, I accept responsibility for the total fee for the session regardless of attendance, including but not limited to discontinuation by expulsion.

Ten Installments Plan Agreement(Required)

I understand if paying in ten installments, my first installment will be charged upon registration and I authorize Debra Collier's School of Dance to charge my debit/credit card for the subsequent 9 installments on the 1st day of each month, beginning September 1, 2025 and concluding on May 1, 2026. By submitting this form, I accept responsibility for the total fee for the session regardless of attendance, including but not limited to discontinuation by expulsion.
