I understand each dancer is limited to two class absences for the summer session, and any dancer exceeding this limit will be expelled from Dance Etc.

Performance Eligibility Policy: Please read thoroughly.(Required)

I understand that for my child to participate in a performance, they must attend the class and dress rehearsal(s) immediately preceding the event.

Performance Attire and Makeup- Please select one shirt and one short option.

Summer DE II: Select the payment option of your choice. Payments will be drafted from the debit/credit card on file so be sure it is up to date in the Parent Portal.(Required)

Two Installments: $124

One Payment Plan Agreement(Required)

Please charge the card on file. Note: If registering online, your account will be charged the smallest installment rate. The remaining balance will be charged within 48 hours of registration. By submitting this form, I accept responsibility for the total fee for the session regardless of attendance, including but not limited to discontinuation by expulsion.

Two Installments Plan Agreement(Required)

I understand if paying in two installments, my first installment will be charged upon registration and I authorize Debra Collier's School of Dance to charge my debit/credit card on June 15, 2025 for the 2nd installment. Note: If registering online, your account will be charged the smallest installment rate. The remaining balance of the 2 installment plan amount will be charged within 48 hours of registration. By submitting this form, I accept responsibility for the total fee for the session regardless of attendance, including but not limited to discontinuation by expulsion.

Three Installments Plan Agreement(Required)

I understand if paying in three installments, my first installment will be charged upon registration and I authorize Debra Collier's School of Dance to charge my debit/credit card on June 15, 2025 for the 2nd installment and July 15, 2025 for the 3rd installment. By submitting this form, I accept responsibility for the total fee for the session regardless of attendance, including but not limited to discontinuation by expulsion.
