I assume full responsibility medially and financially for my child in case of emergency or illness on a trip to the CNADM convention on November 5 and 6, 2022. I also grant permission for medical treatment in case of emergency or illness and do not hold Debra Collier, Debra Collier's School of Dance, Inc., Kristina Collier, Claire Kois, Tammy Seibold or any adult acting in the capacity of a chaperone responsible for accidental harm or illness.
